The fourth costal cartilage is longer and narrower than its predecessor. It ascends obliquely upward from the fourth rib to attach to the sternum via a synovial joint. The pectoralis major muscle attaches on its medial aspect anteriorly, while the transversus thoracis muscle attaches to its posterior surface.
